Individual Insurance Changes
All insurance changes must be made within 30 days of any life-changing event, such as a birth, death, marriage or divorce. For example, individuals who have a child must add the new child to insurance within 30 days of the birth. Individuals needing to make a change should contact Stacy Foster, assistant treasurer, at 330-505-2800 ext. 119 or email [email protected].
Employees who wish to include a spouse in their insurance coverage are required to complete the Spousal Coordination of Benefits form.
Insurance Open Enrollment
Open enrollment for insurance will be held during the month of November. Employees wishing to make changes in insurance should contact Stacy Foster, assistant treasurer, at 330-505-2800 ext. 119. or email [email protected] Insurance coverage for individuals who are newly enrolled will begin January 1.
