Individual Insurance Changes
All insurance changes must be made within 30 days of any life-changing event, such as a birth, death, marriage, or divorce. For example, employees who have a child must add the new child to insurance within 30 days of the birth. Employees who wish to include a spouse in their insurance coverage are required to complete the Spousal Coordination of Benefits form. Insurance Open Enrollment
Open enrollment for insurance will be held during the month of November. Employees wishing to make changes in insurance should contact Stacy Foster, assistant treasurer, at (330-505-2800 ext. 119) or email ([email protected]). Insurance coverage for individuals who are newly enrolled will begin on January 1.
