Pupil Activity Permit
Individuals must apply for or renew pupil activity permits online through ODE's online licensure process.
  • To complete an application, you must have an OH|ID account. To create an OH|ID account, visit the ODE website at www.education.ohio.gov and click on the word LOGIN that appears in small type at the top right of the ODE homepage. The OH|ID account must be active before you can complete an online renewal or apply for any license. It may take several days to activate anOH|ID SAFE account. You will receive an email when the OH|ID account is active.
  • To renew a license or work in an OH|ID Account, it is best to use one of the following browsers: Mozilla Firefox, Google Chrome or Internet Explorer 8, 9 or 10.
  • Background checks must be current and on file at ODE. Complete information about background checks is available on the TCESC website at www.trumbullesc.org under human resources. 
  • Electronic payment is required to complete the online transaction. ODE accepts MasterCard, VISA or electronic checks.
  • The permit’s issue date is important. Please select the school year for which you need to be licensed.
  • The application requires a signature. Applicants working in the following districts should select Trumbull County ESC: Bristol, Champion, Lordstown, and Maplewood. Applicants working in any other school district should select their school district.  
  • Individuals who do not possess another educator credential will receive a three-year pupil activity permit. Individuals who possess a four-year resident educator license will receive a four-year pupil activity permit. Individuals who possess any type of five-year educator license will receive a five-year pupil activity permit. The cost of the permit is $15 per year of the permit term.

Pupil Activity Permit Requirements
  • The school district must verify that pupil activity permit applicants have completed all required trainings and maintain verification of the completion of those training in the school district. This includes current first aid, concussion, CPR training, the Fundamentals of Coaching course and the Sudden Cardiac Arrest course.
  • The Sudden Cardiac Arrest course is an annual requirement that must be documented in each school district. Each year, coaches must review the Ohio Department of Health's new video and related fact sheet. Although districts may choose how they will verify completion, the Department encourages use of its Learning Management System (LMS). Access LMS through individual OH|ID accounts. On the LMS homepage at top right, select the Course Catalog. Then search for sudden cardiac arrest to find the course. After permit holders view the materials and pass a short test, they can earn their certificates of completion. Individuals who are having difficulty accessing the LMS may review the video and related materials on the Ohio Department of Health website. Districts should prepare a simple form for these applicants to sign that will acknowledge their completion. 
  • If the Trumbull County Educational Service Center (TCESC) is signing the application, the athletic director must provide the Pupil Activity Permit Application Checklist to TCESC.
Courses

Concussions in Sports: Ohio's Return-to-Play Law
 
Effective April 26, 2013, pursuant to Ohio Revised Code section 3319.303, any applicant for a new or renewal pupil activity permit is required to complete training specifically focused on recognizing the signs and symptoms of concussions and head injuries and brain injury management as a condition of obtaining a new or renewed permit.

There are two online-courses available free of charge regarding concussions in sports that will meet the requirements of the law. The courses may be completed online in about an hour or less. Individuals are required to complete one of these two course options.
 
 
 
 A certificate of completion may be printed out upon successful completion of either training. An applicant should upload an electronic a copy of the certificate when applying for the pupil activity permit.

The NFHS offers a variety of courses for coaches and advisors of extracurricular activities. The courses are available on the NFHS website.
